Request a Change of Address

As a retired member, you can request the change of your home address by downloading and submitting a Change of Address Form.

If your monthly check is mailed to a financial institution, please indicate on the Change of Address Form whether or not you wish to continue having the check mailed to the financial institution. If you wish to continue having your check mailed to a financial institution, your new home address will only be used for communication and other correspondence from the Retirement System.
